Adult Foster Care (AFC) provides in-home support for individuals with medical or mental health conditions who need daily assistance. With the help of a live-in caregiver, patients receive personalized care in a safe, home-based setting, promoting independence while ensuring their health and well-being.

Our Adult Foster Care program is CARF-accredited, ensuring high-quality, person-centered care. This recognition reflects our commitment to safety, ethical standards, and continuous improvement, providing families with trusted, compassionate support.

Who Qualifies?

Age:

Patients must be 16 years of age or older. ​

MassHealth Eligibility:

Patients must be eligible for MassHealth Standard or MassHealth CommonHealth. 

Medical/Mental Condition:

The patient must have a documented medical or mental health condition that necessitates daily supervision or hands-on assistance with essential tasks. This could include conditions such as physical disabilities, cognitive impairments, neurological disorders, or chronic illnesses that affect independence.

Need for Assistance:

You must require assistance with at least one Activity of Daily Living (ADL), such as bathing, dressing, eating, toileting, or transferring.

 

Live-in Caregiver:

The Adult Foster Care program mandates that the patient must have a live-in caregiver who is responsible for providing daily assistance and ensuring the patient’s well-being. The caregiver may be a family member (excluding a spouse or legal guardian) or a non-related individual who meets program qualifications.

Massachusetts Residency:

The patient must be a resident of Massachusetts to receive AFC services. This ensures that they are eligible for state-funded healthcare programs and can receive care from licensed providers within the state.

What does a patient need to obtain services?

Primary Care Provider (PCP) Order Form:

To qualify for Family First services, a patient must obtain a Primary Care Provider Order Form signed by their healthcare provider, confirming the medical necessity of the program.

Recent Physical Exam:

The patient must have undergone a comprehensive physical examination within the past year to ensure their health status is up-to-date and accurately documented.

Tuberculosis (TB) Screening:

A TB screening is required to rule out any active infections and safeguard the health of both the patient and the caregiver.

MDS Assessment:

A Registered Nurse must complete a Minimum Data Set (MDS) assessment to evaluate the patient's functional capabilities and determine their eligibility for care.

Authorization from MassHealth:

Approval from MassHealth is necessary to confirm that the patient meets financial and medical criteria for Adult Foster Care services.

Qualified Setting & Caregiver:

The patient must reside in a home environment that meets safety and program requirements and have a caregiver who is qualified to provide daily support and supervision.
